Component: AIR BAGS:FRONTAL:DRIVER SIDE INFLATOR MODULE

Problem Summary: Mazda North American Operations (Mazda) is recalling certain model year 2003-2008 Mazda6 vehicles manufactured May 29, 2002, to May 5, 2008, 2004-2008 RX-8 vehicles manufactured April 10, 2003, to February 18, 2008, and 2006-2007 Mazdaspeed6 vehicles manufactured August 4, 2005, to June 29, 2007. The affected vehicles are equipped with a dual-stage driver frontal air bag that may be susceptible to moisture intrusion and other factors, including manufacturing variability that, over time, could cause the inflator to rupture.

Consequence: In the event of a crash necessitating deployment of the driver's frontal air bag, the inflator could rupture with metal fragments striking the driver or other occupants resulting in serious injury or death.

What Owners Should Do: Mazda will notify owners, and dealers will replace the driver's frontal air bag inflator, free of charge. The manufacturer has not yet provided a notification schedule. Owners may contact Mazda customer service at 1-800-222-5500. Mazda's number for this recall is 8215F. Note: The remedy for this recall is not yet available. In the meantime, Mazda will install a new production air bag inflator in these same vehicles under safety recall 15V-345. The interim-remedy air bag inflator will be of the same specification as the original air bag inflator to reduce the amount of time that the air bag inflator has been exposed to humidity. However, the interim air bag inflator does not eliminate the long-term potential for rupture and will only be installed as a stop-gap measure until the newly designed air bag inflators can be produced. Owners need to have their vehicle remedied under recall 15V-345 as soon as possible and, when the new air bag inflators are available, have their vehicles remedied again under this recall.

Problem Summary: Mazda North American Operations (Mazda) is recalling certain model year 2003-2006 Mazda6, and model year 2004-2005 RX-8, and MPV vehicles originally sold, or ever registered, in geographic locations associated with high absolute humidity. Specifically, vehicles sold, or ever registered, in Alabama, Florida, Georgia, Hawaii, Louisiana, Mississippi, Texas, Puerto Rico, Saipan and Guam. Upon deployment of the passenger side frontal air bag, excessive internal pressure may cause the inflator to rupture.

Consequence: In the event of a crash necessitating deployment of the passengers frontal air bag, the inflator could rupture with metal fragments striking the vehicle occupants potentially resulting in serious injury or death.

What Owners Should Do: Mazda will notify owners, and dealers will replace the front passenger air bag inflators, free of charge. The recall began February 4, 2015. Owners may contact Mazda customer service at 1-800-222-5500. Mazda's number for this recall is 8114L. Note: This recall supersedes safety recall 14V-344 as to the front passenger air bag inflators.

Problem Summary: Mazda North America Operations (Mazda) is recalling certain model year 2004-2008 Mazda6, 2006-2007 Mazdaspeed6, and 2004-2008 RX-8 vehicles. Upon deployment of the driver side frontal air bag, excessive internal pressure may cause the inflator to rupture.

Consequence: In the event of a crash necessitating deployment of the driver's frontal air bag, the inflator could rupture with metal fragments striking the driver or other occupants resulting in serious injury or death.

What Owners Should Do: Mazda will notify owners, and dealers will replace the air bag inflators, free of charge. The recall began October 17, 2014 for those vehicles originally sold, or currently registered, in Florida, Puerto Rico, and Hawaii. All other affected owners are expected to be notified starting on February 4, 2015. Owners may contact Mazda customer service at 1-800-222-5500. Mazda's number for this recall is 7914J. Note: This recall previously included vehicles being recalled for potential inflator ruptures on passenger frontal air bag inflators. Those inflators and vehicles are now covered by safety recall 14V-773.

Problem Summary: DOPE, INC. IS RECALLING 42,540 COMBINATION CORNER AND BUMPER LAMP ASSEMBLIES OF VARIOUS PART NUMBERS SOLD FOR USE AS AFTERMARKET EQUIPMENT FOR VARIOUS PASSENGER VEHICLES. THESE HEADLAMPS FAIL TO CONFORM TO THE REQUIREMENTS OF FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 108, "LAMPS, REFLECTIVE DEVICES, AND ASSOCIATED EQUIPMENT." THESE LAMPS DO NOT CONTAIN THE REQUIRED AMBER SIDE REFLECTORS.

Consequence: DECREASED LIGHTING VISIBILITY MAY RESULT IN A VEHICLE CRASH.

Problem Summary: THULE, INC. IS RECALLING 2,323 OWNER-INSTALLED ROOF-MOUNTED FIT KITS, MODELS 3068 AND 3069, MANUFACTURED BETWEEN JANUARY 1, 2008 AND FEBRUARY 28, 2009 THAT ARE USED WITH THULE PODIUM FOOT PACKS, MODELS 460 AND 460R. THESE FIT KITS WERE SOLD AS AFTERMARKET EQUIPMENT FOR USE ON THE VEHICLES LISTED ABOVE. THE BOLT(S) USED TO SECURE THE FOOT TO THE VEHICLE WAS MANUFACTURED TO A HARDNESS THAT MAY CAUSE THE BOLT(S) TO BE BRITTLE AND POTENTIALLY BREAK UNDER STRESS.

Consequence: THIS MAY RESULT IN THE BOLT(S) SNAPPING OFF DURING USE OR WITHOUT A LOAD. THE UNSECURED RACK AND LOAD CAN THEN COME LOOSE WHILE DRIVING CREATING A POTENTIAL ROAD HAZARD, POSSIBLY RESULTING IN A VEHICLE CRASH.

What Owners Should Do: THULE WILL MAIL A RETRO FIT KIT ALONG WITH REPAIR INSTRUCTIONS TO OWNERS FREE OF CHARGE. THE SAFETY RECALL IS EXPECTED TO BEGIN ON OR BEFORE APRIL 13, 2009. OWNERS MAY CONTACT THULE TOLL-FREE AT 1-800-238-2388.
